Common questions

What is Form I‑693?
It is the medical examination form USCIS asks green card applicants for. A designated civil surgeon fills it in and seals it. You file it with your application.
Should I open the envelope?
No. Leave it sealed. USCIS needs the envelope unopened, and we give you a copy of the form for your own records.
What does it cost, and what should I bring?
Call us on (714) 759-4100. We will go through the price and exactly what to bring before you come in.
Do you give the vaccinations here?
No. We check your vaccination records during the exam and tell you exactly which shots USCIS still needs from you. You get those at a pharmacy or from your regular doctor, then bring the record back to us and we complete your form. Your blood tests are drawn here in the clinic.
Which languages do you speak?
English, Spanish and Farsi.
