Workplace health services help employees start and return to work safely.Our comprehensive offerings include:

● Workers’ compensation evaluations

● If your employer requested your visit: Bring a completed and signed employer authorization form (listing the services requested) and a valid photo ID.
● If you scheduled your own visit: Bring a valid photo ID and a method of payment.
● If your employer is receiving results: They will receive them securely by mail or
through their designated portal.
● If you are receiving results directly: You will receive them securely by mail or through
a patient portal.
Turnaround times:
● Same day for non-lab and in-house lab results.
● 3–5 business days for send-out lab results.
● Note: Employees must bring the correct authorization form to ensure sameday results.
● Employer-requested visits: The employer covers the cost.
● Self-scheduled visits: You will be responsible for payment at the time of service.
